Mountain Laurel Integrated Healthcare

  • Mountain Laurel Integrated Healthcare (MLIH) is seeking a Case Manager for our ACT (Assertive Community Treatment) Program. This position will work primarily between Logan and Mingo Counites in West Virginia. ACT Case Manager serves a population of chronic mentally ill, some of whom have dual diagnoses, by providing the basic core services of case management (including service planning, linkage, referral, assessment, advocacy, and crisis support as needed).

  • Mountain Laurel Integrated Healthcare (MLIH) is seeking a Team Leader for our ACT (Assertive Community Treatment) Program. This position works collaboratively with transdisciplinary team members in order to meet integrated health needs of the population served by the team. This population consists of chronic mentally ill patients, with behavioral health and/or dual diagnoses. Act Team Leader provides clinical, organizational, and administrative leadership to the ACT Team.
